Description
The Walking, Mobility & Support Diary is a practical seven-day record designed to help parents and carers document how a child’s condition or disability affects their ability to walk and move around—both indoors and outdoors.
Each day provides structured space to record walking attempts, distances, time taken, stops, refusal, distress, pain, fatigue, balance difficulties, falls and immediate safety risks. Detailed pages allow carers to explain the prompting, supervision, physical guidance, equipment or other support provided, why that help was necessary and what may have happened without it.
A two-page summary helps identify patterns across typical, better and more difficult days. The completed diary may be useful when preparing for professional appointments or providing supporting information for a child’s DLA application or renewal.
