Most endometrial cancers are classified as endometrioid adenocarcinomas, which tend to grow relatively slowly and respond well to treatment when detected early. Less common but more aggressive types include serous carcinoma, clear cell carcinoma, and carcinosarcoma, which require more intensive management due to their higher risk of spread.

 

Risk factors include prolonged exposure to estrogen without adequate progesterone balance, obesity, diabetes, polycystic ovary syndrome (PCOS), and late menopause or early menstruation. A significant inherited risk is associated with Lynch syndrome, which also increases the likelihood of colorectal and other cancers. Hormone replacement therapy containing estrogen alone may also contribute to risk in some individuals.

 

The most common symptom is abnormal uterine bleeding, particularly postmenopausal bleeding, which should always be evaluated promptly. Other symptoms may include pelvic pain, painful intercourse, and unusual vaginal discharge. Because these signs often appear early in disease progression, they can lead to earlier diagnosis compared to other cancers.

 

Diagnosis typically involves transvaginal ultrasound to assess endometrial thickness followed by an endometrial biopsy to obtain tissue for microscopic examination. In some cases, hysteroscopy or dilation and curettage (D&C) may be performed for more comprehensive sampling. Imaging studies such as CT or MRI are used to determine whether the cancer has spread.

 

Treatment most commonly involves surgical removal of the uterus (hysterectomy), often along with removal of the fallopian tubes and ovaries. Depending on the stage and risk features, lymph node assessment, radiation therapy, chemotherapy, or hormone therapy may also be recommended to reduce recurrence risk.

 

Advances in molecular classification have improved understanding of endometrial cancer, allowing clinicians to tailor treatment based on genetic and biomarker profiles. Targeted therapies and immunotherapy are increasingly used in advanced or recurrent cases, particularly for tumors with specific molecular characteristics such as mismatch repair deficiency.

 

Living with endometrial cancer involves recovery from surgery, management of hormonal changes, and ongoing follow-up care to monitor for recurrence. Emotional support, pelvic health rehabilitation, and survivorship planning are important aspects of care. With early detection and appropriate treatment, many individuals experience excellent long-term outcomes and quality of life.
